Sports physicals

Montclair High School will be hosting Sports Physicals here at Montclair in the small gym on Monday May 5th from 3:30pm to 4:30pm.
 
Make sure you Sports Waiver form in addition to the Physical form along with $15 cash.  The forms and directions are on the website as well, CLICK HERE.
